REFERRAL PROCESS
For any new referrals, please follow the process outlined below. Feel free to contact our office with any questions.
To Schedule Evaluation:
1. Generic Dispensing Order
2. Basic Patient Demographics
After Evaluation:
1. Detailed Written Order submitted to MD for signature
2. Diabetic Verification Form submitted for signature (DM Shoes Only)
3. Dear Physician Letter recommending verbiage required for
insurance authorization purposes submitted to MD as
suggested documentation
After Delivery:
1. Update report submitted back to MD
2. Follow-up Schedule Established
